For the Foreign Domestic Worker 6-Month Examination, the documents required are:
If the MOM’s medical screening form states that a Chest X-Ray is required, your helper will be required to attend the nearest partnering Imaging Center to perform the X-Ray.
Our clinic staff will assist with booking the appointment during the screening visit.
Do note that transportation costs to the partnering centres are not included in the screening package.
Our clinic staff will submit the 6ME medical form and the laboratory results to MOM on your behalf.
You will receive an SMS with the submission reference number. If you would like to retain the physical copy of the medical form you can collect it from the clinic. If not collected, forms will be discarded after 2 weeks.
You can change the date and time of the appointment depending on the availability of the slots. Please ensure that screening is done at least 2 weeks prior to the due date on 6ME form.
This is to allow a safe timeframe for a repeat blood test or chest x-ray for any abnormal test results if required.
