- MOM Medical Form (hardcopy)
- Work Permit Card (physical card)
What if my Helper requires a Chest X-Ray to be performed?

Who will submit the completed medical form to MOM?
Our clinic staff will submit the 6ME medical form and the laboratory results to MOM on your behalf.

Can I change the date of the appointment?
You can change the date and time of the appointment depending on the availability of the slots. Please ensure that screening is done at least 2 weeks prior to the due date on 6ME form.
